GI is a top-4 employer cost driver, and it is easy to miss

  • Top Cost Driver

GI, liver, and pancreatic disease account for more than $135 billion in annual U.S. healthcare spend

Gastrointestinal conditions are the #1 cause of treat-and-release emergency room visits. This cost remains hidden in claims data due to structural coding limitations: a single patient with IBS may generate claims for nonspecific abdominal pain, behavioral health issues, or general ER visits, masking the true underlying GI cost driver. We are going to show you what an actual solution looks like, and give you a framework for telling the difference between one that works and one that is well marketed.

The Care ModelGI care the way it should be. One connected team delivering integrated medical, dietary and behavioral care.

Traditional GI care was not built for a national, virtual-first employer population. Effective care requires coordination across four functions.

  • GI medical provider: diagnoses conditions, prescribes medication, orders labs, coordinates procedures
  • GI specialized dietitian: addresses dietary drivers with GI-specific, evidence-based protocols
  • Behavioral health clinician: trained in gut-directed therapies that address the brain-gut connection
  • Care coordinator: manages transitions and follow-up so patients do not fall through the gaps

This is the validated model we built Oshi around, and it is the model we recommend you use to evaluate anyone else in this category.

7% decrease in GI medical spend while other medical costs rose 14%

  • Client Results

A recent case study of a 10,000+ life pediatric health system found that just 3% enrollment in virtual GI care moved GI medical spend down 7% while all other medical costs rose 14% over the same period, based on matched control claims conducted by Accorded. The result was $11,637 in per-member all-cause cost savings, an 8.6x ROI. Two enrollment decisions made this level of participation possible, a waived cost-share and a proactive employee awareness campaign. Employers do not need mass enrollment to see population-level GI savings.

Four questions that separate a credible GI program from a well-marketed one

This simple framework can pressure-test any GI program you’re considering. We built it for our consulting partners to rigorously evaluate vendors, including us, so you can recommend solutions with confidence.

  • Vendor white papers and proprietary ROI calculators are not a substitute for a propensity-matched cohort analysis conducted by a payer or actuarial firm. Ask for the methodology and the independent source.

  • A program that cannot diagnose or prescribe is a support tool, not a care delivery model. Ask if the care is being delivered by employed clinicians or is the program administered by contract providers and health coaches.

  • Satisfaction scores alone are not sufficient for a purchasing decision. Ask for claims-based utilization results showing reductions in the real cost drivers in GI, ER visits, imaging, hospitalizations, and surgery.

  • A GI benefit that half a percent of eligible employees use will not move population spend. Ask about the variables that convert per-member savings into population-level impact like cost-share waiver support and co-marketing infrastructure.
